How to patch-test properly, and what a reaction actually looks like

Everyone is told to patch-test. Almost nobody does, partly because the instruction is usually given without explaining what you are actually watching for or how long it takes.

Why "natural" makes it more important, not less

Botanical extracts and essential oils are among the more frequent causes of contact dermatitis. Plants produce these compounds as defences — they are biologically active by design. A herbal product is not automatically gentler than a synthetic one; it is simply differently sourced.

This is worth saying plainly because the assumption runs the other way: people patch-test a drugstore serum and slather a homemade turmeric pack on without a second thought.

How to do it properly

1. Apply a small amount to the inside of your forearm — thin skin, easy to observe, out of the way.
2. Leave it. Do not wash the area.
3. Check at 24 hours, and again at 48 if you have reacted to things before.
4. If nothing happens, use it normally.

The 24-hour wait is the part people skip. Irritant reactions can appear within minutes, but allergic contact reactions are delayed — commonly 12 to 48 hours. A five-minute test tells you almost nothing.

What a reaction looks like

• Redness that persists rather than fading in a few minutes.
• Itching, which is the more telling sign — irritation tends to sting or burn, allergy tends to itch.
• Small raised bumps, or tiny blisters.
• Skin that feels hot to the touch in that patch alone.

Mild transient tingling on application is not necessarily a reaction. Redness still present the next morning is.

When to test even if you never do otherwise

Before anything goes near your eyes. Before a product covers a large area, like a body oil. Whenever you are pregnant or breastfeeding. And on anyone under about twelve, whose skin barrier is thinner.

If you do react

Stop, wash the area with plain water, and leave it alone. No scrubbing, no "pushing through it". If the reaction spreads beyond the test patch, or involves swelling or difficulty breathing, that is a doctor — immediately, not tomorrow.
